Submitting the Americo Application
Submit new business applications by faxing the applications with the Faxed Application Transmittal Form (#AFSFAX2002) to 800.395.9261. A maximum of eight applications may be sent per transmittal form. Please retain the original application(s) for your files - do not mail them to Americo.
